Please fill out all required details, but do not sign or date the document in advance. The grantor must sign in the physical presence of the notary and witnesses.
The grantor (and any witnesses you bring) must present unexpired, government-issued photo identification (such as a Passport, Driver’s License, or PR Card).
If possible, bring two eligible, independent witnesses who meet Ontario's legal requirements. If you do not have witnesses, please notify us in advance so we can assist with arranging independent witnessing.
